Job Summary:

The Director of Case Management/Retention will oversee and coordinate all aspects of case management and client retention. This role involves developing and implementing strategies to ensure exceptional care for clients while maintaining high levels of satisfaction and loyalty. The ideal candidate will possess strong leadership skills, a background in case management, and a passion for delivering outstanding care services.

Key Responsibilities:

  • Lead, mentor, and supervise the case management team to ensure effective and efficient client care.
  • Develop and implement client retention strategies to enhance satisfaction and loyalty.
  • Monitor and evaluate case management practices to ensure compliance with industry standards and company policies.
  • Collaborate with healthcare professionals, clients, and families to address and resolve any issues or concerns.
  • Analyze client feedback and retention metrics to drive continuous improvement initiatives.
  • Maintain up-to-date knowledge of industry trends and best practices in home care and case management.
  • Prepare and present reports on case management performance and client retention to senior management.

Qualifications:

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field (Master’s degree preferred).
  • Proven experience in case management and client retention, with a minimum of [X] years in a leadership role.
